Clarithromycin is a macrolide antibiotic used to treat (and sometimes prevent) certain types of bacterial infection, particularly of the skin or respiratory system. Patients also buy Clarithromycin to help treat duodenal ulcer disease, and to prevent recurrence of the ulcers.

This medication can be used in patients of any age, though the young, elderly, and individuals with other major medical conditions will require adjusted dosage.

Patients more often buy Clarithromycin as the brand name Biaxin, though it's also available as a generic.

When to Use Clarithromycin

As an antibiotic, Clarithromycin destroys certain types of bacteria. In some parts of the world there are concerns about antibiotic resistance building up against Clarithromycin, though in other locations resistance is nearly unheard of. Clarithromycin is applicable against:

  • Haemophilus influenzae
  • Haemophilus parainfluenzae
  • Streptococcus pneumoniae
  • Mycoplasma pneumoniae
  • Chlamydophila pneumoniae
  • Moraxella catarrhalis
  • Streptococcus pyogenes
  • Staphylococcus aureus
  • Mycobacterium avium
  • Mycobacterium intracellulare
  • Helicobacter Pylori

Which in turn may cause the following infections:

  • Sinusitis
  • Pneumonia
  • Tonsillitis
  • Skin infections
  • Otitis media
  • Bronchitis
  • Duodenal ulcer disease

Note that the above bacteria are not the only bacterial strains that cause the same, or very similar, infections; patients are advised to have the offending bacteria identified---by a doctor or independent lab---before they buy Clarithromycin to treat them. Likewise, the medication will provide no benefit in viral or fungal infections.

Taking Clarithromycin

Patients can buy Clarithromycin in one of three formulations; tablet, extended-release tablet, and granules for an oral solution. Tablets and the oral solution are largely the same; dosage and duration of treatment are the same, and either can be taken with or without food according to personal preference. Extended-release tablets are taken just once every 24 hours, and should be taken with food.

Antibiotics work best when at consistent levels in the body, so effort should be made to take doses at the same times each day.

Treatment, and duration of treatment, will depend on a number of factors. Most significant among them is the condition being treated and whether one is using oral solution/regular tablets or the extended-release tablets.

In otherwise healthy adults, treatment with oral solution/regular tablets is:

  • Bronchitis caused by M. catarrhalis or S. pneumoniae - 250 mg every 12 hours, taken for 14 days.
  • Bronchitis caused by H. influenzae - 500 mg every 12 hours, taken for 14 days.
  • Bronchitis caused by H. parainfluenzae - 500 mg every 12 hours, taken for 7 days.
  • Pneumonia - 250 mg taken every 12 hours for 14 days; when caused by H. influenzae, treatment should be 7 days. H. parainfluenzae and M. catarrhalis should only be treated with extended-release tablets.
  • Sinusitis - 500 mg taken every 12 hours for 14 days.
  • Tonsillitis - 250 mg taken every 12 hours for 10 days.
  • Otitis media - 250 mg taken every 12 hours for 7 to 14 days.
  • Skin infections - 250 mg taken every 12 hours for 7 to 14 days.

Extended-release tablet dosing is much simpler, but they're not recommended for treatment of every condition:

  • Bronchitis - 1 gram taken every 24 hours for 7 days.
  • Pneumonia - 1 gram taken every 24 hours for 7 days.
  • Sinusitis - 1 gram taken every 24 hours for 14 days.

Parents who plan to buy Clarithromycin for children should be aware that dosing and duration of treatment differs from that of adults. Treatment is usually done with oral solution, and dosing is typically 15 mg per kg of body weight per day, divided into two 12 hour doses. Treatment for nearly all conditions in children will extend for 10 days. There are, however, a lot of mitigating factors and doctor instruction may vary.

Treating ulcers---more specifically, duodenal ulcers---is done with other medications. Most commonly used is amoxicillin, along with either lansoprazole or omeprazole.

The standard dose with these medications is 500 mg Clarithromycin every 12 hours for 10 to 14 days. To reduce risk of recurrence, standard treatment is 500 mg every 8 hours for 14 days. 1 gram of amoxicillin is also usually administered, along with 30 mg lansoprazole.
